Technical Assistance:
Training hours need to be completed by end of November.

Comments:
An unannounced renewal inspection was conducted at 410 PM. There were 7 children with 2 staff for a total of 22 points. The Provider and the assistant were observed feeding the children and playing with the children. Hand washing and food prep was observed. The children had materials to play with and space to run around. The Provider stated they use the back yard for play. Files, and special care provisions were reviewed as well. Standard #: 22VAC40-111-750-B
Description: Based on observation, the Provider has not stored topical skin products in area inaccessible to the children. Evidence: 1. Diaper changing area is in a room where children were observed playing. The Inspector observed two containers of Desitin cream located on shelves below the changing table.

Plan of Correction: Removed during the visit.

Standard #: 22VAC40-111-60-B
Description: Upon review of 5 children's files, the Provider has not ensured the paper work is complete and/or current. Evidence: 1. Child #1's file has not been updated and signed by the parent within the last year. 2. Child #2's file only contains 1 of the 2 required Emergency Contacts. 3. Child 3 and 4 are siblings and are missing the following information: child 3A has not been updated and signed by the parent in the last year. Child 3B does not have address of Emergency Contacts.

Plan of Correction: We will continue to request information from each family.

Standard #: 22VAC40-111-360-B
Description: Based on observation, the Provider has not placed small bags of trash into garbage containers. The Inspector observed two bags of trash that were tied closed but sitting on the kitchen floor.

Plan of Correction: I will place in trash cans.

Standard #: 22VAC40-111-540-B
Description: Based on observation, the Provider is using a walker. Evidence: 1. Inspector observed all of the children's equipment and noticed an infant walker next to a swing. 2. The Provider said she did not realize it could not be used.

Plan of Correction: We will remove it today.
