Technical Assistance:
Provider is not MAT certified and is not administering medication.

Comments:
An unannounced inspection was conducted at 230. The Inspector arrived just as 9 children were waking from nap. The Provider and an assistant were present and the children were observed preparing for snack, playing, and one child was going home because the child did not feel well. The children made the Inspector some tea and snack from their play kitchen and talked to the inspector about a pumpkin patch. The Inspector reviewed records but could not observe outside due to rain. Standard #: 22VAC40-111-60-B
Description: Upon review of a child #1's file, the inspector found the file was missing the following information: parent's work address, medical policy number, and Provider's signature that medication is not administered.

Plan of Correction: I will go over with the parent and sign my part of the medication form.

Standard #: 22VAC40-191-40-C-1-b
Description: Based on record review, the Provider has not obtained the Central Registry within 30 days of employment for assistant #2. Staff member started September 12, 2016 and the file has documentation to show it was sent.

Plan of Correction: I sent on time and I hope it will be back soon.
