54.1-3408 Provider must be MAT certified to administer prescription medication.
An unannounced monitoring inspection was conducted from 11:20 am - 12:30 pm. There were 9 children in care with the provider and 1 assistant ages, 21 months, 23 months, 2 years, 2 years, 2 years, 3 years, 3 years, 3 years, and 4 years for a total of 20 points. The children were observed participating in outdoor activities and preparing for and eating lunch. Lunch served today is pizza, lima beans, peaches, and milk. Nine children's records were reviewed and in compliance. The provider and one assistant records were reviewed. Standard #: 22VAC40-191-40-D-1-c Description: Based on observation of the provider record it was determined that not all background checks were repeated every three years. Evidence the most recent background checks and sworn statement or affirmation on record for the provider are as follows: Criminal history check: 5/22/13, Central Registry Check: 5/31/13, Sworn Statement or affirmation: 12/1/2012. Plan of Correction: The provider will complete the sworn statement or affirmation form. The provider will complete and submit for results and updated criminal history check and central registry check. The provider made a clerical error in the reminder system to alert the provider to update the background checks and sworn statement.
