Technical Assistance:
For a child's proof of identification, a hospital letter or passport can be used instead of the birth certificate.

Comments:
An unannounced monitoring inspection was conducted today from 10:45 am - 12:30 pm. There were nine children present with the provider and two assistants for 30 points. I observed two's and toddlers returning from playing outdoors; eating lunch; at play and preparing for nap. I heard them sing the ABC song; listen to a story and pretend to read by handling books and turning pages (an early step in literacy!) If you have any questions, please contact me at brenda.kuhlman@dss.virginia.gov

Violations:
Standard #: 22VAC40-111-70-A
Description: During today's review of children's files, some written information to be provided to parents before the child begins care was observed to be missing in two records. Evidence: Of six files reviewed today, Child # 1 and Child # 2 did not have all required information in the parent agreement such as requirement for the parent to inform the provider within 24 hours if any household member develops a reportable communicable disease; requirement for caregivers to report suspected child abuse/neglect; custodial parents' rights; discipline policies; address of the Dept. of Social Services website; and parental notifications. (This is all on the "Information for Parent's Form.")

Plan of Correction: I will give the form to the parents today.

Standard #: 22VAC40-111-240-A
Description: During today's inspection, some outdoor areas were observed to not be maintained in a safe and operable condition. Evidence: There were two pieces of siding at child height where it was pulling away from the side of the garage, leaving sharp edges exposed. The edge of a gutter alongside the garage was observed being propped up by a thin pole, also in an area accessible to the children's play area.

Plan of Correction: I will put duct tape on the siding today; and I will have my husband fix the gutter.
