- Technical Assistance:
An unannounced renewal inspection was conducted on Tuesday, October 11, 2016 from approximately 10:15am to 12:10pm to determine the provider's compliance with licensing standards. In attendance were 11 non-resident children. The children were being supervised by two caregivers for a total of 29 points. Children were observed playing outside with trikes, swinging, going on a worm hunt and taking bathroom breaks and washing their hands. Infants were observed playing with toys inside. Lunch consisted of pasta, corn, applesauce and milk. The child care areas were filled with supplies and toys for the children. During the inspection a complete walk through of the home was completed. The outdoor play area was inspected.
During the inspection, the licensing inspector reviewed the following: emergency drill logs, emergency supplies including first aid kit, flashlight, battery operated weather radio, smoke detectors, emergency plan, menu, and daily attendance. The provider stated there were no medications belonging to the children in care during the inspection. Transportation is not provided at this time. During the inspection three children's and three providers' records were reviewed.
Please complete the 'plan of correction' and 'date to be corrected' for each violation cited on the violation notice and return it to me within 10 calendar days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the word 'corrected' is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).
The risk profile tool will be used for the licensing recommendation once all of the necessary paperwork is received by the licensing department. The renewal application was received prior to the expiration of the current license. Until the new license is received, the provider is legally operating. Standard #: 22VAC40-111-180-A Description: Based on review of three caregivers' records, one record did not contain a negative tuberculosis screening within the required time frame. Evidence: During the inspection, the licensing inspector observed the record for Caregiver #1 did not contain a current report of a tuberculosis form. The record contained a tuberculosis screening dated 07/07/14. A tuberculosis screening is required to be obtained at least every two years from the date of the first screening. Plan of Correction: On Monday, October 24, 2016 I made an appointment to have TB test done.
Standard #: 22VAC40-111-500-D Description: Based on observations, the provider did not ensure all "S" hooks located on playground equipment were tightly closed. Evidence: During the inspection, the licensing inspector observed a "S" hook with an approximate 1/2 of an inch opening located at the top of the blue swing. Plan of Correction: We switch out swings so each age group has turns on swings. I will switch to a D link.
