Comments:
An announced initial inspection was conducted on September 29, 2016 from 10:10am to 11:05am. There were no children in care during this inspection as the provider only cares for children before and after school. The provider is currently caring for no more than four children at this time. The provider's home has two stories and a portion of the first level is being used for child care. The provider will use the family room and sunroom for care of the children. The family day home is requesting a licensed capacity for 12 children ages four years old through 12 years old. The family day home will only offer before and after school care for school age children when school is in session. The provider will not offer care on the days when school is closed. The provider has been approved by zoning to provide care for this number. All required documents including the menu, emergency phone numbers, daily schedule, emergency preparedness plan were posted. First aid kit and emergency equipment were present. The program will offer meals and snacks to children in care. No medication will be administered. Transportation is not provided. The provider has submitted all of the required application materials at this time. During the inspection, compliance with some standards could not be determined since the center is not yet in operation. Compliance with these standards will be determined during unannounced visits once the program is in operation. Though some standards of care could not be determined during the initial study, the provider is in substantial compliance with the standards that could be determined. A six-month conditional license will be recommended. During the conditional period, the center will be subject to unannounced licensing inspections, with the first inspection taking place within the first sixty days of licensure. Description: Based on a review of caregiver records on 09/29/2016, the provider did not ensure that current certification in cardiopulmonary resuscitation (CPR), is from the American Red Cross, American Heart Association, American Safety and Health Institute, or the National Safety Council, or by a community college, a hospital, a rescue squad, or a fire department. EVIDENCE: Caregiver #1's CPR certification was obtained from National Health Care.

Plan of Correction: Contacted approved trainer today to schedule class as soon as possible.
