An unannounced monitoring inspection was initiated today at 1:30pm. The physical plant, 2 caregiver/household member files, 4 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected today. There were 5 children, totaling 11 points, under the supervision of 2 caregivers. During the inspection, children were observed resting at nap time. Earlier the children played outside to play soccer, read book and danced to music. Today, the children were served ground beef with rice, broccoli, plums, bread and milk. The Provider is currently certified in MAT (medication administration training), CPR and First Aid. However, there are no children with any medication needs at this time. Standard #: 22VAC40-111-100-A Description: Based on file review, 1 out of 4 children's records does not have documentation of a physical examination within 30 days of attendance. Evidence 1. Child #4 began care on 11/25/13. There is no physical on file. Plan of Correction: The Provider will ask the parent to bring this into the FDH.
Standard #: 22VAC40-111-60-A Description: Based on file review, 2 out of 4 children's files have not been updated annually. Evidence 1. Child #2's record was last updated on 10/5/15. 2. Child #3's record was last updated on 9/29/15. Plan of Correction: The Provider will have the parents complete this.
Standard #: 22VAC40-111-90-C Description: Based on file review, 2 out of 4 children's files does not have documentation of additional immunizations every six months under the age of two years. Evidence 1. Children #1 and #2 are 2 years old. They do not have their 2 year old immunization record on file. Plan of Correction: The Provider will ask the parents for this.
