An unannounced monitoring inspection was conducted beginning at 9:30 and ending at 10:55. There were four children in care, ages three and four years. The children were observed in circle time, playing at centers and participating in music and movement activities. The provider was observed using kind voice tones and explaining the reasons for rules. Children listened intently. They were familiar with their daily routines and looked forward to different activities. Two family files were reviewed - see violation notice. Four children's files were reviewed and were found to be in compliance. Emergency escape drills and shelter in place drills are up to date. Standard #: 22VAC40-111-700-A
Description: Based on a review of the Medication Administration Policy that is signed by the parents, it was determined that thepolicy that was signed by the parents did not reflect the actual policy. Evidence: The policy was reported that the provider gave Over The Counter Medication. The provider gives only non prescription topical ointments. It was just a misreading of the form.

Plan of Correction: The provider will adjuste the form for each parent and have them resign.

Standard #: 22VAC40-111-180-A
Description: Based on a review of family files, it was determined that both adults in the home do not have updated TB tests. Evidence: Family member #1 and #2 have Tb tests that have not been repeated after two years. #1 had a Tb test that should have been repeated by 4/29/16b and #2 by 4/30/16.

Plan of Correction: TB tests will be done and in the future will will be done before expiration.
