63.2 Liability Insurance Disclosure
Today an unannounced renewal visit was conducted with the Provider. This visit took place in the morning between 9:30am-12 there were 7 children with the Provider and 1 assistant for a total of 28 points within the supervision guidelines. The children were observed having morning outdoor free play, play-doh, blocks, tummy time, lunch and nap time. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and provider records, fire drill log, attendance, menu and emergency procedures were observed. Standard #: 22VAC40-111-960-E Description: Based on observation of the bottles and infants present, the bottles did not include date. Evidence: 4 Infants were present and none of the bottles included the date. Plan of Correction: Provider stated that the parent brings the bottles everyday however she will inform them or make sure they have dates.
Standard #: 22VAC40-111-100-B Description: Based on a review of the children records, two records did not include documentation of a physical examination within 30 days of enrollment. Evidence: Child #3 & #6's record did not include documentation of a physical exam. Plan of Correction: Provider will notify the parents today to request a copy.
Standard #: 22VAC40-111-110-A Description: Based on a review of the children records, the records did not include the documentation of a physical & immunization on the Virginia Department of Health form. Evidence: Child #3 & #6's record did not include documentation that the child was adequately immunized. Plan of Correction: Provider will notify the parent today and request that the form be completed.
