Comments:
An unannounced renewal inspection was completed on 11/4/16. There were four children present for after school care with the provider supervising. The point total was four. One newly enrolled child's record was reviewed. The provider's record and household records had been reviewed at the initial inspection and were current. The inspector arrived at the provider's home at 3:45 pm. The provider had to leave to pick up the children from school and returned at 4:20 pm. The inspector departed the provider's home at 5:30 pm. The children were observed during arrival, during snack, during free play time, and during departure. There was discussion with the provider about the following: ensuring children are checked in and marked off the attendance within the required time frame (30 minutes). The provider gave the inspector the renewal application and FDH Addendum form during the inspection.

Violations:
Standard #: 22VAC40-111-920
Description: Based on observation and interview with the provider, the provider failed to ensure that the posted menu listed substitutions. Evidence: 1. The afternoon snack menu for the date of the inspection listed graham crackers and juice. The children were served "cuties" (oranges) and chocolate pudding. The provider did not change the menu during the inspection.

Plan of Correction: The provider will ensure that the menu is changed when substitutions are made in the future.

Standard #: 22VAC40-111-1010
Description: Based on observation and interview with the provider,the provider failed to ensure that the requirements for the vechicle during transportation of the children were followed. Evidence: 1. A child who is seven years old was being transported without a car or booster seat. The child will be eight years old tomorrow (11/5/16). The Standards and the Virginia state law requires that each child up to eight year of age is in an individual car seat or individual appropriate restraint in accordance with the law.

Plan of Correction: The provider will ensure proper transportation is provided in the future.

Standard #: 22VAC40-111-990-B
Description: Based on observation and interview with the provider, the provider failed to ensure that the required items were in the vehicle used to transport the children during the inspection. Evidence: 1. The following required items were missing from the vehicle used to transport the children from school in the afternoon to the provider's home: the name, address, and telephone number of the family day home; a copy of the parent's written permission to transport the child; a copy of each child's emergency contact information as required in 22 VAC 40-111-60 B 2; emergency supplies as required in 22 VAC 40-111-760 (first aid kit with ice pack).

Plan of Correction: The provider has all of the required information and supplies in the vehicle normally used for transporting the children which was not available for inspection. The provider had to use substitute transportation on the date of the inspection because there were no keys available for the vehicle normally used.

Standard #: 22VAC40-111-60-B
Description: Based on record review, the provider failed to ensure one of one newly enrolled child's record contained complete information. Evidence: 1. The record of child #8 was missing the mother's complete work address and the child's medical/health insurance policy number.

Plan of Correction: The provider will talk with the parent over the weekend and have the information documented by Monday of next week.
