An unannounced monitoring inspection was conducted on 11/7/2016 from 10:00am - 12:00pm. At the time entrance, there were were eight children in care with two caregivers. The ages of the children counted for a total of 20 points. Children were observed during morning program time and lunch. Hand washing was reviewed prior to lunch. Records were reviewed for eight children, the provider, assistants and one household member.

Standard #: 22VAC40-111-920
Description: Based on observation, the provider failed to ensure the menu for the current one-week period shall be posted or placed in an area accessible to parents. Evidence: The menu for the previous week (10/30/16 - 11/4/16) was posted when the inspector arrived at 10:00am.

Plan of Correction: The current menu will be posted today.

Standard #: 22VAC40-111-60-B
Description: Based on record review, the provider failed to ensure that a child's record shall contain the following information: *Name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached; *Documentation of annual review of the child's emergency contact information. Evidence: 1. The record for child 1 lacked an address for the two designated emergency contacts. 2. The record for child 2 and child 3 was last reviewed and updated by the parent on 7/5/2015. 3. The record for child 4 was last reviewed and updated by the parent on 7/8/2013.

Plan of Correction: The parent of child 1 will be asked to provide an address for both emergency contacts this afternoon. The parents of child 2, 3, and 4 will be asked to review and update, if applicable, the emergency information in enrollment records this afternoon.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on record review, the provider failed to obtain for a household member a search of the central registry before three years since the date of the last central registry finding. Evidence: 1. The most recent central registry finding for one household member was conducted on 5/14/2012. a) A request had been requested and received by CPS on 10/14/2015. The request had been returned to the provider for additional information however, the request was not resubmitted for completion.

Plan of Correction: The central registry check will be re-submitted today.
