Comments:
Your time and assistance were appreciated during the unannounced renewal inspection conducted on November 4, 2016 between the hours of approximately 10:45 a.m. and 1:33 p.m. Eleven children were enrolled and nine children were in the care of two caregivers on this date (25 points). Five children's records were reviewed and/or updated. The provider's record, assistant's record and household members' records were up-to-date and, therefore, not reviewed/updated. No medications were administered, as is the policy of this family day home. Facility documents (attendance form and emergency evacuation and shelter-in-place drill record), posted information (evacuation diagram, menu, emergency telephone numbers, daily schedule, license and licensing inspection documentation) and safety equipment (first aid kit, flashlight, radio and fire extinguisher) were observed/reviewed. Program observation was completed. Activities observed on this date included: indoor play; music, songs and movement; stories; television; coloring; lunch; naptime and pick-up/departure of two children. Adult/child interactions, behavior guidance and daily routines/transitions were also observed. The activities and lunch provided were consistent with the posted daily schedule and menu. On this date, lunch included macaroni and cheese, pears, green beans and milk. Substantial compliance with the licensing standards was maintained. Violations cited during the licensing inspection conducted on August 8, 2016 were reviewed for compliance; no repeat violations were cited. Current exceptions to compliance were noted on the violation notice. The licensing standard regarding water temperature was discussed. Please continue to monitor the temperature of the water, to ensure it consistently measures between 105 degrees F and 120 degrees F. Standard #: 22VAC40-111-80-A
Description: Based on review and/or update of five children's records and discussion with caregiver #1 on November 4, 2016, the program failed to obtain verification of age/identity for one enrolled child. Evidence: 1. Review of child E's record, on this date, determined that verification of age/identity had not been obtained from the child's parent within seven days of the child's first day of attendance (10/18/2016). 2. Caregiver #1 confirmed that age/identity verification had not been obtained.

Plan of Correction: The information has been requested from the child's parent and will be obtained. In the future, verification of age/identity will be obtained within seven days of a child's first day of attendance or local authorities will be contacted.

Standard #: 22VAC40-111-90-A
Description: Based on review and/or update of five children's records and discussion with caregiver #1 on November 4, 2016, the program failed to obtain documentation of immunizations before one child attended the family day home. Evidence: 1. Review of child E's record, on this date, revealed that documentation of the child's immunizations was not included in the child's record (first date of attendance - 10/18/2016). 2. Caregiver #1 verified that the child's immunization record was not on file.

Plan of Correction: Documentation of immunizations has been requested and will be obtained. In the future, documentation of immunizations will be obtained before a child's first day of attendance.
