Comments:
An announced initial inspection was conducted on Thursday, October 20, 2016 from 9:48am to 11:02am. A follow up visit was conducted on Friday, October 21, 2016. There were two children in care during this inspection. The family day home is requesting a licensed capacity for 12 children ages 12 months through 12 years old. The provider has been approved by zoning to provide care for this number. All required documents including the menu, emergency phone numbers, and emergency evacuation and shelter in place routes were posted. The family day home has two levels with the family room and dining room on the main level being used for childcare. The provider has two dog who live in the home as well. Both dogs are up to date on their required vaccinations. The first aid kit and emergency equipment were present and inspected. The program will offer snacks and meals to children in care. No medication will be administered. Transportation is provided. The provider has submitted all of the required application materials at this time. During the inspection, compliance with some standards could not be determined since the family day home is not yet in operation. Compliance with these standards will be determined during unannounced visits once the program is in operation. Though some standards of care could not be determined during the initial study, the family day home is in substantial compliance with the standards that could be determined. A six-month conditional license will be recommended. During the conditional period, the center will be subject to unannounced licensing inspections, with the first inspection taking place within the first sixty days of licensure. Standard #: 22VAC40-111-760-A-1
Description: Based on observation on 10/20/2016, the provider did not ensure that the first aid kit contained the minimum items. EVIDENCE: 1. The first aid kit in the home did not have two triangle bandages. 2. The first aid kit in the van that transports children did not have two triangle bandages and a thermometer.

Description: Based on observation on 10/21/2016, the provider did not ensure that the required first aid kit contained an ice pack or cooling agent. EVIDENCE: The first aid kit in the van that is used for transportation did not have an ice pack.

Standard #: 22VAC40-111-800-A
Description: Based on a review of documentation on 10/20/2016, the provider did not ensure that the family day home has a written emergency preparedness and response plan. EVIDENCE: The family day home did not have a written emergency preparedness plan.

Standard #: 22VAC40-111-140-B-3
Description: Based on a review of caregiver records and caregiver interview on 10/20/2016, the provider did not ensure that they had a current certification in cardiopulmonary resuscitation (CPR). EVIDENCE: Caregiver #1 did not have documentation of certification of CPR. Caregiver #1 stated that they had completed the class, however did not their CPR card or certificate.

Plan of Correction: Will contact instructor for a duplicate certificate or redo certification.

Description: Based on a review of caregiver records and caregiver interview on 10/20/2016, the provider did not ensure that they had a current certification in first aid. EVIDENCE: Caregiver #1 did not have documentation of certification in first aid. Caregiver #1 stated that they had completed the class, however did not their first card or certificate.

Plan of Correction: Will contact instructor for duplicate certificate, or redo certification.

Standard #: 22VAC40-111-330-A
Description: Based on observation on 10/20/2016, the provider did not ensure that a landline telephone, excluding a cordless or cell phone, shall be available, operable, and accessible during the family day home's hours of operation. EVIDENCE: The family day home does not have a working phone number for a land line telephone.

Plan of Correction: Will get turned on within the next 2 days.
