Discussed with provider crib compliance, regarding cribs manufactured prior to June 28, 2011.

Comments:
Today an unannounced monitoring inspection was conducted from 11:35am-12:40pm. There was 1 child ( totaling 3 points) directly supervised by 2 staff. The home, 2 staff records, 2 children?s records, evacuation drills, and policies were inspected. There were no medications on site today. The child in care was observed playing freely and having lunch. Hand washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice. Description: Based on record review, emergency evacuation procedures were not practiced monthly with all caregivers and children. Evidence: 1. The last record evacuation drill was performed on 3/1/2016.

Description: Based on record review, the family day home did not keep a written record of children in attendance each day. Evidence: 1. The attendance sheet available for review, last recorded children in care on 3/31/2016.
