Comments:
On September 20, 2016, an unannounced inspection was conducted with local agency personnel in response to a complaint concerning forbidden actions. Interviews were conducted and records reviewed. The available evidence gathered during the investigation does not support the allegation, therefore, the complaint is determined to be not valid. Standard #: 22VAC40-111-520-A
Complaint related: Yes
Description: Based on observation and provider interview, the inspector found that there were not enough cribs available for the number of infants present. Evidence: At the time of the inspection, three infants were present and only two cribs were available.

Plan of Correction: Provider took the third crib out of storage area and placed it in the sleeping area.

Standard #: 22VAC40-111-560-A
Complaint related: Yes
Description: Based on observation, the provider did not provide direct care and supervision of the children in care. Evidence: 1. At the time of the inspection, the inspector observed an eight month old infant sleeping in a seat, alone in a room with the door closed. No baby monitor was available. 2. At the time of the inspection, the inspector also observed three children, ages 12 months, 16 months, and 23 months left alone in the downstairs room with the door closed, when the provider came upstairs to answer the door.

Plan of Correction: Provider will place a baby monitor in the sleeping area. Provider will leave the basement door open when answering the door in the future if assistant is not present.
