An unannounced monitoring inspection was initiated today at 12:40pm. The physical plant, 3 caregiver/household member files, 4 children's files, caregiver training, children's activities, emergency procedures, drills and supplies were inspected today. There were 6 children, totaling 14 points, under the supervision of 2 caregivers. During the inspection, children were observed eating lunch, cleaning up, having diapers changed and enjoying free play. The interactions between the children and caregivers were seen to be positive and constructive in nature. The children bring their lunches from home each day. The Provider is not certified in MAT (medication administration training) and there are no children with any medication needs at this time. Both caregivers are certified in CPR and First Aid. Standard #: 22VAC40-111-120-B Description: Based on file review, a caregiver's file does not have documentation that two or more references as to character and reputation as well as competency were checked before employment. Evidence 1. Caregiver #2 only has 1 completed reference check on file. Plan of Correction: A 2nd reference check will be completed.
Standard #: 22VAC40-111-70-A Description: Based on file review, 4 out of 4 children's files do not have policies pertaining to the administration of medications at the FDH. Plan of Correction: The Provider will give the parents a Medication Policy form to sign.
