An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 12:30 PM and departed at 2:15 PM. Eleven children, totaling 29 points, were in care with two caregivers. Nap time and transition to afternoon activities were observed. A sample size of four children's records were reviewed. It was reported that no medication is administered to children in care and no children's injuries were reported. Caregiver annual training was reviewed. An exit meeting was conducted with the provider prior to closure of the inspection. Please contact the inspector if there are any questions.
Standard #: 22VAC40-111-60-B Description: Based upon review of four children's records, the provider has not ensured that each child's record contains the names, addresses and telephone numbers of two designated persons to be contacted in an emergency when a parent cannot be contacted. Evidence: The record provided for child 2 had a name and telephone for only one designated person to be contacted in an emergency. There was no address indicated for the designated person. Plan of Correction: The provider stated that the parent will be asked to provide the required information.
