Comments:
An unannounced monitoring inspection was conducted from 10:30 a.m. to 12:15 p.m. At the time of entrance, 5 children, ages 12 months to 3 years old, for a total of 15 points, were in care with 2 caregivers present. The records sample size consisted of 5 children?s records and 6 provider / assistant/ household member records. Children were observed playing on a covered outdoor deck, having lunch which consisted of chicken nuggets, mashed potatoes, broccoli, applesauce and milk and resting quietly on mats at nap time. Medication administration was not observed. The provider states that no medication is administered to children in care, children are not routinely transported in a vehicle and there have been no accidents or injuries requiring first aid or medical treatment incurred by children in care since the last inspection. Areas of non-compliance are identified on the violation notice. Thank you for your time during today's inspection. Please do not hesitate to contact me if I may be of assistance. The licensee provided verbal responses to the violations during the inspection, which were entered on to the violation notice by the Licensing inspector.

Violations:
Standard #: 22VAC40-111-70-A
Description: Based on a review of records, the licensee did not ensure that each enrolled child's record contains parent acknowledgement of the written polices required to be provided to parents before the first day of attendance. Evidence: There was no written acknowledgement available documenting that child #1's parents were given the family day home's required written information to include Operating information including the hours and days of operation, holidays or other times closed, and the telephone number where a message can be left for a caregiver; Schedule of fees and payment plans; Check in and check out procedures; and Policies for the administration of medications.

Plan of Correction: The provider inadvertently left these forms that capture this information out of this file. She states that she will provide this information to child #1's family and have the parent sign the two forms indicating acknowledgement.
