Discussion was held on the topics of current enrollment, current staffing, and that the documentation of all Proof of Identifications should only be made by the Provider and/or Assistant and not the child's parent. The Provider also makes copies from the original birth certificates or other acceptable proof of id's.
The following inspection took place between the hours of 10 am through 12 pm (noon). Today, I met with the Provider on site at her Family Day Home (FDH) located in Ashburn, Virginia. At the start of this unannounced renewal inspection there were a total of 9 children (25 points) with 2 caregivers. A total of 9 children?s files and 4 staff/household member?s files were reviewed. The children were engaged in the following: toy play, movie time, music, dance, story time, infant sleeping, lunch time, bathroom breaks, hand washing, nap time, and other organized activities. Standard #: 22VAC40-111-90-C Description: Based on review, 1 of 9 children's files reviewed indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for a child (who was not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years. Evidence - 1. On the date of inspection (12/01/2016), a current immunization record (within the past 6 months) was not available for Child #9. 2. The most recent immunization documentation on file for Child #9 was dated - 05/16/2016. 3. Based on the information on file, on the date of inspection, Child #9 was under the age of two years. Plan of Correction: An updated immunization page shall be obtained during the two year check-up for Child #9.
