Thank you for your time and assistance with the unannounced monitoring inspection conducted today, December 7, 2016. The on-site inspection was conducted from 12:40 p.m. to 4:00 p.m. On this date, there were six (6) children in the care of the provider and two (2) assistants for fifteen (15) points. There were a total of twelve (12) children enrolled in the program. There were three (3) children's records reviewed and the records of three (3) caregivers and one (1) household member were reviewed. The provider, two caregivers and three (3) children were interviewed. There were no medications being administered at the time of the inspection, which is the policy of the provider. The exceptions to compliance were noted on the violation notice and the risk ratings for the violations were noted on the supplemental page. Lunch was observed to meet USDA guidelines. Outdoor play was not observed. There were an ample supply of age appropriate toys and equipment. Choice time, bathroom and hand washing procedures, infant feeding, lunch, and nap time were observed during the visit. Standard #: 22VAC40-111-460-C Description: Based on review of records and verification from the provider, one household pet did not have a current vaccine for rabies. Evidence: 1. The last rabies vaccine that had been obtained for Pet #2 was 10/18/2015 and it expired on 10/18/2016. 2. The provider confirmed the vaccine had expired. Plan of Correction: The provider will ensure that the dog gets a new rabies as soon as possible.
Standard #: 22VAC40-191-40-D-1-c Description: Based on record review and verification from the provider, it was determined that three caregivers and one household member did not obtain background checks within the last three years. Evidence: 1. For Caregiver #1, the date of the last criminal history record check was 03/26/2013 and the date of the last search of the central registry was 02/22/2013. 2. For Caregiver #2, the date of the last criminal history record check was 02/09/2013 and the date of the last search of the central registry was 02/22/2013. 3. For Caregiver #3, the date of the last criminal history record check was 05/20/2013 and the date of the last search of the central registry was 05/31/2013. 4. For Household Member #1, the date of the last criminal history record check was 02/09/2013 and the date of the last search of the central registry was 02/22/2013. 5. Caregiver #2 verified that that it had been more than three years since the background checks had been updated. Plan of Correction: The provider will complete the requests for the background as soon as possible.
