Comments:
The following inspection took place between the hours of 10am through 11:45am. Today, I met with the Provider on site at her Family Day Home (FDH) located in the town of Herndon, Virginia. At the start of this unannounced monitoring inspection there were a total of 9 children (46 points) with 3 caregivers. A total of 4 children?s files and 3 staff/ 1 household member?s files were reviewed. The children were engaged in the following: circle time, coloring, singing the days of the week and watching a movie. Violations:
Standard #: 22VAC40-111-780-B
Description: Based on review of children's records, an annual review of the emergency contact information. Evidence: Child #3's record did not contain an annual review of emergency contact information

Plan of Correction: Provider will give the file to the parents of Child #5 to review the emergency contact information for updating and signing.

Standard #: 22VAC40-191-40-D-5-a
Description: Based on review of household records, a central registry check was not found. Evidence: Household member #1 who is over the age of 14 did not have a central registry check.

Plan of Correction: Household member !1 is 14 1/2 years old. Provider will run the central registry check for her on 12/06/16.
