An unannounced renewal inspection was conducted today, December 13, 2016 to determine the providers' compliance with the State Licensing standards. The inspector arrived at approximately 10:00 am and concluded the inspection at 1:45 am. Upon the inspectors arrival, three children were in care with the one provider for a total of eight points. Children were observed sorting bean bags by color. Later children danced to the song, "Shake My Sillies Out" and "Freeze". The provider interacted in a positive manner with children. All areas of the family day home were inspected including the outside play area and deck. The home has age and stage appropriate toys and materials in operable condition for children in care. Non-medical emergency supplies, first aide kit and the emergency preparedness plan were all reviewed and checked for compliance. The family day home had the daily attendance, schedule, menu, License, and emergency numbers posted. Smoke detectors and fire extinguisher were checked. Two caregiver records and three children's records were reviewed during today's inspection. Medication is not being administered. The last documented emergency evacuation drill was held on 11/20/2016. The two required annual shelter-in-place drills were completed on 02/07/2016 and 07/14/2016. The provider does not provide transportation. There are no pets at the family day home. Today's lunch consisted of baked chicken, applesauce, broccoli, wheat bread and milk. The violations from the previous inspection on January 12, 2015 were checked for corrections. There was one repeat violation cited during today's inspection. Standard #: 22VAC40-111-780-B
Description: Based on review of children's records, the provider did not obtain the parents signed acknowledgement of the annual review of the emergency contact information to ensure the information is correct. Evidence: The most recent signed acknowledgement of review of emergency contact information for child #2 was dated 02/18/2014, exceeding the annual requirement.

Plan of Correction: Per the Provider: I will get Dad to review and sign the record tonight.

Standard #: 22VAC40-111-100-A
Description: Based on review of children's records, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: Child #2, enrolled 02/18/2014, did not have documentation of a physical on record.

Plan of Correction: Per teh Provider: I will tell the parent ASAP, and have her get a copy for the record.

Standard #: 22VAC40-111-90-C
Description: Based on review of children's records, the provider did not obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: The most recent immunizations for child #1, a 14 month old, was dated 03/11/2016 exceeding the six month allowance to obtain documentation of additional immunizations.

Plan of Correction: Per the Provider: I will tell the parent to get a copy of the updated immunizations for the child's record.

Standard #: 22VAC40-111-430-A
Description: Based on observations, the provider did not ensure children under two years of age did not have access to stairs. Evidence: Child #1, who is under the age of two years, was observed in the living room on the main level of the house. Two sets of stairs are accessible to the living room. A set of stairs leads to the third floor and the other set leads to the basement level. A gate was observed at the bottom of the stairs leading into the basement however there was no gate at the top of the staircase.

Plan of Correction: Per the provider: I will purchase and install the gate at the top of the stairs.
