Technical Assistance:
Clarified with provider appropriate toys and equipment in and around cribs. Provider expressed interest in becoming a CDC provider. Provider encouraged to complete the Phase I CDC trg online and the Phase II training provided by licensing.

Met with provider today to complete an unannounced Renewal Inspection. Provider present along with two assistants. 11 children present today for a total of 40 points. Observation of diapering was made along with morning playtime. Infants observed napping and transition to play time. Arrived for inspection at 10:15am to 11:45. Standard #: 22VAC40-111-750-B
Description: Based on walk thru of provider's home, topical skin products were found accessible to children. Evidence: Provider's diaper changing station had diaper cream under diaper changing station which was accessible to children.

Plan of Correction: There were no topical skin products present under the diaper changing station. There was indeed diaper cream was stored away in a drawer out of sight of the children. Nonetheless, I will place lock on diaper changing station to prevent access by the children. (sic)

Standard #: 22VAC40-111-960-E
Description: Based on walk thru of provider's home, bottles were found not labeled Evidence: Three bottles were found in the refrigerator downstairs not labeled with the child's name and date.

Plan of Correction: I will make sure to have parents place names on bottles of children every day. (sic)

Standard #: 22VAC40-111-60-B
Description: Based on review of children's records policy number of a child's insurance policy was not found. Evidence: Child #1's record did not contain the policy number for their insurance policy.

Plan of Correction: Parents have already provided information on insurance policy as requested. (sic)

Standard #: 22VAC40-111-320-A
Description: Based on walk thru of provider's home a small appliance was found plugged in. Previously cited on 03/02/2016. Evidence: Provider's coffee maker was found plugged in and not in use.

Plan of Correction: The coffee maker was subsequently disconnected. (sic)

Standard #: 22VAC40-111-510-C
Description: Based on walk thru of infant room cribs were not spaced the required 12 inches apart. Evidence: Two cribs occupied were not spaced 12inches apart as required.

Plan of Correction: One of the cribs was empty. Nonetheless, the two cribs were separated apart to provide the clearance requested. (sic)
