The licensing inspectors conducted an unannounced renewal inspection from 10:00am - 12:15pm. There were nine children in care for a total of twenty two points with two caregivers. The children were observed at free play and during lunch. Three caregiver's and four children's records were reviewed. Violations from the previous inspection were also reviewed and one was repeated. Standard #: 22VAC40-111-60-B Description: Based on record reviews, the family day home failed to obtain all required information for the children's records. Evidence: 1. The inspector reviewed four children's records. Two of four records reviewed (children's records #2 and #4) did not contain all required information for their emergency contacts. Child record #2 had information for only one emergency contact and child record #4 did not have physical addresses for their emergency contacts. Child record #4 did not have information for the parent's employment. Plan of Correction: The provider will request this information from the children's parents and review the children's files to make sure all information is complete.
Standard #: 22VAC40-111-180-A Description: Based on record reviews, the family day home failed to have caregiver's obtain repeat tuberculosis screenings. Evidence: 1. The inspector reviewed three caregiver's records. Two of three records reviewed (caregiver's records #1 and #3) contained tuberculosis screenings that were over two years old. The screenings were dated 03/24/2014. Plan of Correction: The provider will have the caregivers have repeat tuberculosis screenings completed when they close for the holidays. The provider will send copies of the screenings to the licensing inspector.
Standard #: 22VAC40-111-460-C Description: Based on interview, the family day home failed to have documentation of their pet's vaccinations. Evidence: 1. The inspector asked the provider for documentation of vaccinations for their pet. The provider said the records were at her assistant's home and were not available. Plan of Correction: The assistant will send a copy of the pet's vaccinations to the licensing inspector. The assistant will bring the copies back to the provider's home so they will be available for review.
