Comments:
An unannounced renewal inspection was conducted today, December 15, 2016 to determine the providers compliance with the Licensing standards. The inspector arrived at approximately 12:30 pm and concluded the inspection at 2:30 pm. Upon the inspectors arrival, four children and one caregiver were present, with a total point value of eleven points. Two children were finishing their lunch while two children were napping. Later a parent of one child was notified by the provider that the child was not feeling well. The provider made sure the child had plenty of water to drink and kept the child calm with books while waiting for the parent. Today's lunch consisted of chicken, bread, mixed vegetables, fruit cocktail and milk. The provider interacted with children in a positive manner. All areas used for the family day home were inspected. The home has age and stage appropriate toys and materials in operable condition for children in care. The first aid kit along with non-medical emergency supplies for the home and vehicle, and the emergency preparedness plan were all reviewed and checked for compliance (see violation notice). The family day home had the daily attendance, schedule, menu, License, emergency numbers, and evacuation map posted. A smoke detectors and a fire extinguisher were observed. The inspector interacted with both the provider and children in care. Two caregiver records, one household members record and four children's records were reviewed during today's inspection. The provider has current MAT certification, however the provider stated that currently their is no medication on site. The provider and assistant have current CPR and first aid training. The last documented emergency evacuation drill was held on 11/30/2016. One of the two required shelter-in-place drills was documented on 08/16/2016. A second shelter-in-place drill will need to be completed by the end of the year along with the December evacuation drill. The provider provides transportation. There are no pets at the family day home. A complete renewal application has been received by the Department. Standard #: 22VAC40-111-780-B
Description: Based on review of children's records, the provider did not obtain the parents acknowledgement of review of the child's emergency contact information to ensure the information is correct. Evidence: The most recent review of the emergency contact information for the record of child #4 was dated 09/17/2015, exceeding the annual requirement.

Plan of Correction: Per the Provider: I will have the parent review and sign this evening.

Standard #: 22VAC40-111-800-B
Description: Based on review of the emergency preparedness plan, the provider did not review the emergency plan at least annually and document in writing each review and update to the emergency plan. Evidence: The most recent documented review/up-date to the family day home emergency plan was dated 11/17/2011, exceeding the annual requirement.

Plan of Correction: Per the Provider: I will review the emergency plan tonight and document it. I will also set a calendar reminder on my computer and phone for every eleven months to alert me to review it.
