Comments:
An unannounced monitoring inspection was conducted from 11:15 am - 12:45 pm. There were eleven children in care being supervised by the provider and two assistants age, 4 months, 6 months, 12 months, 15 months, 15 months, 2 years, 2 years, 2 years, 2 years, 3 years, and 4 years for a total of 32 points. The children were observed eating lunch and preparing for nap. Lunch is provided from home. Eleven children's records were reviewed. Violations:
Standard #: 22VAC40-111-960-E
Description: Based on observation of four infant bottles it was determined that not all bottles were labeled with first, last name, and date. Evidence: Four bottles were observed that were not labeled in a manner to identify the owner.

Plan of Correction: The provider will label the bottles with first, last name and date.

Standard #: 22VAC40-111-100-A
Description: Based on observation of eleven children's records it was determined that not all records obtained documentation of a physical within 30 days of enrollment. Evidence: Child A with a start date of 9/5/16 did not have documentation of a physical within 30 days of enrollment.

Plan of Correction: Administration will request documentation of a physical from the parents of Child A.

Standard #: 22VAC40-111-90-A
Description: Based on observation of eleven children's records it was determined that not all record's obtained documentation of immunizations before the first day of enrollment. Evidence: Child A with a start date of 9/5/16 did not have documentation of immunizations on file.

Plan of Correction: Administration will request documentation of immunization from the parent of child A.
