An unannounced monitoring inspection was conducted today from 11:00 - 12:30. There were 6 children = 20 points with two, then three caregivers. The children played with toys, balls, empty boxes and listened to stories and ate lunch. The exit interview was conducted with the licensee. The violation notice will be e mailed for completion.
Standard #: 22VAC40-111-100-A Description: Based on a review of children's records, the licensee did not obtain a physical examination for three children within 30 days of their beginning attendance. Evidence - The records for children # 3, 4 and 6 did not contain physical examinations. Plan of Correction: I have told the parents
Standard #: 22VAC40-111-60-B Description: Based on a review of children's records, the licensee did not obtain all of the required information. Evidence - The record for child #1 did not contain a house number for emergency contact #2. The record for child #6 did not contain a street address for emergency contact #1. Standard #: 22VAC40-111-590-A Description: Based on observation, the licensee did not place one infant on his back in the crib. Evidence - A five month old infant was observed being placed on his tummy in a crib twice. Plan of Correction: I understand, the infants should [sic] on their back to asleep [sic] in a crib. I will do it.
