An unannounced monitoring inspection was conducted with provider. There were five day care children and two resident children present during today's inspection totalling 8 points. The school age children arrived from school early today and the younger children were still resting. The children were dancing to music on the computer. Three children's files were reviewed - please see Supplmental Summary for missing items. Reminder: first aid and CPR certification are due to be repeated before 3/7/2017. There are no children receiving medication at day care today - provider has Medication Administration Training certification valid until 11/13/2018. Standard #: 22VAC40-111-100-A
Description: Based on review of three children's files, the provider did not have a physical examination on file for a child within 30 days of child's first attendance. Evidence: Child A (date of enrollment 6/8/2015) does not have documentation of a physical on file.

Plan of Correction: Provider will request the information from the parent again.

Standard #: 22VAC40-111-90-A
Description: Based on review of three children's files, the provider did not have documentation of an immunization record on file for each child before the child's first day of attendance. Evidence: Child B and Child C (date of enrollment 12/19/2016) do not have documentation of an immunization record on file.

Plan of Correction: Provider requested the information from the parent at today's pick up.
