Technical Assistance:
Guidance provided on getting children to settle to sleep. Blankets should not be draped around that faces of children being held even under close supervision.

Comments:
An unannounced monitoring inspection was conducted today between 1:45pm and 2:45pm. At the start of this inspection there were a total of 5 children (11 points) with 1 caregiver. A total of 5 children?s files and 1 staff/household member?s files were reviewed. The inspector observed children settling down for nap. Emergency plans, preparedness materials, the outdoor play space and medication policies were reviewed. If you should have any questions, please call Kara Wright at (703) 537-6241.

Standard #: 22VAC40-111-60-B
Description: Based on record review, one child's file was incomplete. Evidence: 1. Child # 2's record contained one of two required names, addresses and telephone numbers of designated persons to contact in case of an emergency if the parent cannot be reached. 1. Child #1's record was not reviewed annually by parents. The last dated review occurred on 3/6/2015.

Standard #: 22VAC40-111-480-B
Description: Based on observation, not all equipment was not appropriate for use by a two year old. Evidence: Child #5 was strapped into an infant rocker and proceeded to walk around the napping area while belted into the chair and hunched over with the chair on his back.

Standard #: 22VAC40-111-510-E
Description: Based on observation, rest mats were not sanitized as needed. Evidence: 1. After a child has urinated on herself at nap time, the provider changed the child's clothes, removed the soiled clothes and flipped the soiled mat so that the soiled side faced the floor without cleaning or sanitizing. 2. The child was then put back to rest on the mat.

Standard #: 22VAC40-111-600-B
Description: Based on observation, a toddler spent more than 30 consecutive minutes confined in a chair. Evidence: 1. Child #5 was observed strapped into an infant rocking chair from at the beginning of the inspection 1:45pm until the end of the inspection at 2:40pm. 2. The provider stated that she places the toddler in the chair to fall asleep where upon she will move him to a cot.
