The portable electric heater shall continue to be used only in the space not occupied by children in care. Discussed the placement of the diaper trash can.
An unannounced monitoring inspection was conducted today. The provider and one assistant caregiver were caring for 3 children, totaling 11 points. This one more caregiver than is required by the Minimum Standards. The children were observed playing with toys, interacting with each caregiver, and preparing for lunch. The children on solid foods were to be served spaghetti, chicken, oranges, salad, and milk for lunch today. This inspection was completed between approximately 10:45 am and 12:00 pm. Standard #: 22VAC40-111-920 Description: Based on observations, it was determined that the provider did not update the posted menu as required. Evidence: The provider did not serve all of the foods indicated on the posted menu today. The provider did not update the menu to reflect the lunch food substitutions. Plan of Correction: I will remember to update the menu when necessary.
Standard #: 22VAC40-111-940-A Description: Based on observations, it was determined that the refrigerator temperature was not maintained at 40 degrees or below. Evidence: The thermometer inside the refrigerator used to store some of the children's food and drinks registered a temperature of approximately 45 degrees. Plan of Correction: I adjusted the temperature in order to keep the food from freezing inside the refrigerator. I will re-adjust the temperature and make sure it stays at 40 degrees.
Standard #: 22VAC40-111-960-E Description: Based on observations, it was determined that three infant bottles were not labeled and stored as required. Evidence: Three infant bottles of formula were not labeled with each child's name. In addition, two of these bottles were placed on the counter instead of inside the refrigerator after being partially finished by each child. Plan of Correction: I will create a better system to make sure all of the bottles are labeled. All bottles will be placed in the refrigerator when the children are finished drinking.
Standard #: 22VAC40-111-120-B Description: Based on a review of caregiver records, it was determined that one record was not complete. Evidence: The file for the assistant caregiver does not include: verification of her age; emergency contact information; two completed reference checks; documentation of completed Orientation Training. Plan of Correction: The file will be updated and complete.
Standard #: 22VAC40-111-60-B Description: Based on a review of children's records, it was determined that two records were not complete. Evidence: 1. Child record #1 does not include: the mother's work phone number; the parent's medical insurance information. 2. Child record #2 does not include: a signed "Declaration of Liability Insurance" document; a signed "Medication Policy" document. Plan of Correction: The files will be updated and complete.
Standard #: 22VAC40-111-530-F Description: Based on observations, it was determined that one infant was sleeping with soft bedding and items in place around him. Evidence: One 3month old infant was observed sleeping with a thick blanket in his crib swing. This child also had a bib in place while asleep. Plan of Correction: The blanket and bib were removed during the inspection.
Standard #: 22VAC40-111-590-B Description: Based on observations, it was determined that one infant was not moved to his crib after falling asleep in a small crib swing. Evidence: Upon arrival, the Inspector observed one 3month old child soundly asleep inside a small crib swing. This child was not moved to his crib after falling asleep in the swing. Plan of Correction: All babies will be moved to their cribs if they fall asleep in a different space.
Standard #: 22VAC40-111-680-A Description: Based on observations, it was determined that one caregiver did not wash her hands prior to feeding a child. Evidence: The Inspector observed the assistant caregiver hold a 6month old while feeding him his bottle. This caregiver did not wash her hands prior to feeding the child. Plan of Correction: I will make sure she always washes her hands before feeding any of the children.
