An unannounced renewal inspection was conducted today. Observed 8 children directly supervised by the provider and an assistant. Ratios and supervision were in compliance. A sample of 4 children's records, caregiver records and household records were reviewed. The home, required posted documents, emergency policies and supplies, hand washing and cleaning procedures were also reviewed. No medications were reported on site. The children were engaged in various activities, to include: Circle time, getting ready for a PJ party, practicing letters and singing songs. Babies were napping at the time of inspection so feeding and diaper procedures were discussed since they were not directly observed. The interior of the family day home was found to be clean, organized, and well supplied with toys and equipment for the children. Areas of non-compliance are identified in this report. Description: Based upon record review, required annual review of emergency plan was not completed/documented as required.

Plan of Correction: Nothing has changed, but will document as required.

Standard #: 22VAC40-111-60-B
Description: Based upon record review, required documentation was found to be missing or incomplete. Evidence: Child A's record is missing insurance policy number and complete information required for proof of identity, Child B's record is missing complete information required to document proof of identity authorization for emergency medical treatment signed by a parent. Child C's record is missing complete information required to document proof of identity and authorization for emergency medical treatment signed by a parent. Child D's record is missing documentation of an annual file review by parent.

Plan of Correction: The provider did not realize that the enrollment forms copies cut off signatures needed by parents giving authorizations. Will have all parents resign documents in January as part of a file review.

Description: Based upon record review, documentation of additional immunizations of every 6 months for the first 2 years were incomplete. Evidence: Child A (Date of Birth 2/2016) last immunization record was dated 4/2106, immunization records were due to be updated 10/2016.

Description: Based upon observation, areas in the home were in need of repair. Evidence: In the children's rest room there are holes in the drywall that need to be repaired.
