An unannounced monitoring inspection was conducted today. Five children were present with the provider and her adult daughter. Total 12 points with two caregivers. The children were playing with floor toys, watching a children's TV program and eating lunch. The infant in care was sleeping and being fed. One child is new since the last inspection. His record was reviewed today. The provider plans to renew her MAT certification, but until she does will not administer any medication. Description: Based on record review, no documentation of a current TB screening report could be located for one household member.

Plan of Correction: The TB screening report will be located or a new TB screening report for this household member will be obtained.

Standard #: 22VAC40-111-260-E
Description: Based on observation, it was found that containers full of water were accessible to children on the outside play area. Findings: At the time of the inspection, the inspector observed a small wading pool and a bucket full of water sitting outside where the children play.

Plan of Correction: The containers of water will be emptied today. The provider will ensure there are no containers or a pool of water accessible to children playing outside.

Description: Based on record review, no documentation of a child protective services (CPS) central registry check has been obtained for a household member who turned 14 years old in October 2016.

Plan of Correction: The request for a CPS central registry check for this household member was mailed today.
