The licensing inspector conducted an unannounced monitoring inspection Thursday, January 19, 2017. This was a focused 60-day monitoring visit to determine the family day home's continued compliance since being licensed on October 24, 2016. The children were observed in a variety of activities. Upon the inspector's arrival the children were observed eating a snack and were later observed completing homework. Today, there were seven children, totaling six points, present with two caregivers. Snack consisted of fish sticks, corn, french fries and apple juice. Three children's records and two caregiver records were reviewed during the inspection. The violations from the previous inspection were checked for corrections. There weren't any repeat violations found during today's inspection. Standard #: 22VAC40-111-60-B Description: Based on a review of children's records o 01/19/2017, the provider did not ensure that one of three records reviewed contained the telephone number of each custodial parent's place of employment. EVIDENCE: The record of Child #2 did not have a telephone number listed for the place of employment documented for the child's father. Plan of Correction: Corrected on site. Missing information received from parent during inspection.
