An unannounced renewal inspection was completed on 2/6/2017 beginning at 11:45 A.M. and ending at 2:30 p.m. The licensee was present with the licensee's one infant and grandchild and two child care children. The points were at nine and in compliance. The LI reviewed five children's and one staff and a household member's record during the inspection. Observation of the children during: free play, TV time, art, and snack. Discussion about upcoming changes to the licensee's household, and future plans for day care schedule.

Standard #: 22VAC40-111-60-B
Description: Based on review of five children's records, the license failed to ensure that all records were complete. Evidence: 1. The inspector found that child # 2 with the start date of 5/1/2016 did not have verification of birth documented. The birth certificate number was not documented.

Plan of Correction: The licensee will document the verification of birth and will ensure all information is documented.

Standard #: 22VAC40-111-180-A
Description: Based on review of review of staff's records, the licensee failed to ensure that all TB screening were updated every two years. Evidence: 1. The licensee and the substitute TB screening expired 1/28/2017.

Plan of Correction: The licensee will have new TB screening completed as soon as possible.

Standard #: 22VAC40-111-240-A
Description: Based on observation the licensee failed to ensure that all areas and furnishings of the family day home were maintained in a clean, safe and operable condition. Evidence: 1. The inspector found that the gutter outside the front door in the outside play area was cracked and had sharp edges. 2. The plastic shopping cart is missing a wheel and had sharp plastic edges exposed to the children in care.

Plan of Correction: The shopping cart will be discarded today. The gutter will be fixed as soon as the licensee can get someone to look at it. All areas will be monitored.

Standard #: 22VAC40-111-280
Description: Based on observation in the kitchen area, the license failed to ensure that all chemicals were not stored with food. Evidence: 1. The inspector observed a spray bottle of cleaning chemicals sitting on the kitchen cabinet sitting next to food.

Plan of Correction: The licensee removed the chemical during the inspection and will make sure that all chemicals are not stored with food. The licensee stated the visitor placed the chemical on the counter to clean and did not return it to the storage area.
