Description: Based on observation during the renewal inspection the television in the living room is not secured. Evidence: 1. The flat screen television is sitting on a four by four foot cubbie. The cubbie moves when shaking and could cause the television to fall off of the cubbies onto a child.

Plan of Correction: The provider will check to see if the television can be bolted to the cubbies to prevent it from tipping off. The provider makes sure that the children do not horseplay in the area.

Description: Based on review of records the provider failed to have the chimney inspected annually. Evidence: 1. The last documented chimney inspection was dated in 2012.

Plan of Correction: The provider will continue to look for the inspection and if not able to find it then she will contact the chimney inspector to have another inspection completed.
