Comments:
The licensing inspector conducted an unannounced monitoring inspection on Friday, February 10, 2017 from approximately 9:45 am until 11:35 am to determine the provider's compliance with licensing standards. During this inspection, there were five children present totaling 12 points being supervised by three caregivers. The children were observed in the designated area playing with age appropriate toys, engaged in active play with peers and staff, participating in a Valentine's day coloring activity, and preparing for lunch. The children's lunch today consisted of mozzarella cheese, pizza crust, tomato sauce, jello with fruit, and milk. The designated area for childcare was deemed appropriate and equipped with age appropriate equipment and materials to meet the needs of the children. During the inspection, the licensing inspector interacted with staff and children when appropriate. The inspector observed the required postings to include license, prior inspection summary, and menu. The first aid kit and emergency supplies, posted emergency contacts, fire and shelter-in-place drills, emergency preparedness plan were inspected. During this inspection, three children's, the provider's, the provider's assistant, and a household member's records were reviewed for compliance of standards. There are currently no medications being administered by provider. The licensing inspector completed some but not all licensing standards during this inspection. Standard #: 22VAC40-191-40-D-1-c
Description: Based on review of the caregiver's records, three records did not contain background checks that were updated within the required time frame. Evidence: During the inspection, the licensing inspector observed the record for Caregiver# 1 contained a sworn statement last dated 10/29/13. The record for Caregiver#2 contained a sworn statement last dated 02/28/13. The record for Caregiver #3 contained a sworn statement last dated 10/29/13. Background checks to include sworn statement are required to be updated at least every three years from the previous forms.

Plan of Correction: Sworn statement documents will be completed by staff today.
