Comments:
An unannounced afternoon inspection has been conducted today. There are no children present today except the Provider's own child. The Provider is enrolled in the Vendor Agreement and has been taking the 10 hour program. The Inspector reviewed 3 children's files, first aid kit, and documents posted on the wall. The Provider has corrected most of the violations from the last inspection and has kept files maintained. Please call with questions 703-479-4679

Description: Based on record review of 3 children's files, the Provider did not obtain child #1's emergency contact's addresses.

Plan of Correction: Will talk to parents to ask for addresses.
