Comments:
Today an unannounced monitoring inspection was conducted with Provider. This visit took place in the morning and afternoon between 11:20am and 12:45pm. There were 9 children present with the Provider and 1 assistant for a total of 19 points within the supervision guidelines. The children were observed finishing lunch, transitioning to group time on the carpet with a card game and preparing for nap. Handwashing technique observed. Positive interactions were observed during this inspection. A complete inspection to include 9 children and 2 staff records, and all household member files reviewed today fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations:
Standard #: 22VAC40-111-60-B
Description: Based on review of children's records not all required information was found. Evidence: Child #1 and #2 did not have a second emergency contact and address.

Plan of Correction: Provider has the information and will document in each record.

Standard #: 22VAC40-111-690-D
Description: Based on observation of the diapering area, the area was determined to be out of compliance. Evidence: The diaper changing mat has a tear on the mat and is no longer nonabsorbent.

Plan of Correction: Provider will replace the mat.
