Technical assistance was provided on the following items during the Inspection: 22VAC40-111-(6)-210 (Annual training); 22VAC40-111-(8)-660 (Swimming and wading activities)
An unannounced monitoring inspection was conducted on 2/21/17 from 9:30am - 10:40am. At the time of entrance there were 9 children ages six months old to five years old (23 points) in care with two caregivers. A tour of the home was conducted and children were observed playing inside and eating lunch. Records were reviewed for four children and two caregivers. There is no medication at the family day home. Emergency plan and emergency supplies were reviewed. Areas of non-compliance are identified on the violation notice, and were discussed during the exit interview.
Standard #: 22VAC40-111-770 Description: Based on observation and interview, it was determined that the provider did not ensure there is a a working portable battery-operated weather band radio available to caregivers at all times. Evidence: 1. The weather band radio available for viewing during the inspection was not operable by batteries. It was operational by by use of a hand crank. 2. The provider confirmed that there was not a working battery-operated weather band radio during the inspection. Plan of Correction: The provider responded: When I purchased the current weather radio I was not aware the batteries were not replaceable and it only works in the crank mode. therefore I have purchased a standard battery operated weather band radio.
Standard #: 22VAC40-111-60-B Description: Based on a review of four children's records, it was determined that the provider did not ensure they maintain and keep at the family day home a complete record for each child enrolled that contains all required information. Evidence: 1. The record for child #3, present during the inspection, did not contain the following items: the name and policy number of the child's medical insurance; did not indicate if the child had previously attended child care; the written authorization for emergency medical care; whether or not there is liability insurance; documentation that the parent was provided with provisions of the emergency plan. . 2. The provider reviewed the records for child #3, and confirmed the record was not complete. Plan of Correction: The provider responded: 1. Child #3 Policy number was added, the child previously child-care was added, the emergency medical care was completed, and the parent signed and was provided a provision emergency plan.
Standard #: 22VAC40-111-210-A Description: Based on a review of the staff training log, it was determined that the provider did not ensure that caregivers shall obtain 16 hours of training annually in areas relevant to their job responsibilities. Evidence: 1. The record for staff #2 did not have any documentation of annual training being completed. 2. The provider reviewed the record for staff #2 and confirmed that there was no documentation to demonstrate that staff #2 had completed the 16 hours of trainings required by the standards. Plan of Correction: The provider responded: I have taken action to be sure that the trainings are not overlooked each year from this day forward. With this being said, she has her training completed for 2017.
