Technical Assistance:
Consultation was given on using the backyard for play area, moving the diapering area closer to the bathroom to allow staff and children immediate access to hand washing as well as the foot operated trash can to dispose of soiled diapers and subsidy requirements if the provider enrolls as a vendor in the subsidy program.

Comments:
An unannounced monitoring visit was conducted today. Six day care children plus the provider's daughter were present with three caregivers for twenty three points which is within ratios for supervision. Children were observed in infant care and free play. Basement space has two rooms for children as well as a bathroom and storage area. All food is bought from home and was properly labeled and safely stored (refrigerator was 38 degrees) Two assistant files and one child's record were reviewed as well as emergency drills and crib area. Description: Based on record review, documentation of two reference checks for two assistants was not available. Evidence: Reference checks (2) for both assistants were taken verbally but were not written down and dated to verify that they were conducted prior to employment.

Description: Based on observation, the steps leading into the new day care space do not have protective guard rails. Evidence: The steps leading into the day care space are more than three and no hand rail is attached to the wall to assist young children.

Description: Based on observation, a prohibited piece of equipment is in use. Evidence: A child was observed playing in a walker in the basement play area. Walkers are not allowed in licensed homes.
