Comments:
An unannounced full renewal inspection was conducted today. The provider and one assistant caregiver were caring for 10 children, totaling 31 points. The children were observed playing with toys, singing songs, interacting with each caregiver, and eating lunch. The majority of children were observed eating lunch from home. The provider served a vegetarian lunch consisting of macaroni and cheese, mixed fruit, and milk today. This inspection was completed between approximately 10:15 am and 11:50 am. Violations:
Standard #: 22VAC40-111-120-B
Description: Based on a review of caregiver records, it was determined that one file is not complete. Evidence: The file for assistant caregiver A does not include emergency contact information and documentation of her date of hire.

Plan of Correction: I will have her fill out the cover sheet with this information today.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records, it was determined that three records were not complete. Evidence: 1. Child record #1 does not include: documentation to show that the parent reviewed the child's contact information within the past year. 2. Child record #3 does not include: the father's work phone number. 3. Child record #4 does not include: a signed "Medication Policy" document.

Plan of Correction: I will make sure all missing information is updated.

Standard #: 22VAC40-111-500-C
Description: Based on observations, it was determined that one stationary piece of play equipment was in place over a hard surface. Evidence: The backyard swing set is in place over a hard grass/dirt surface.

Plan of Correction: I will install padding beneath the swings, slide, and climbing equipment.

Standard #: 22VAC40-111-530-F
Description: Based on observations, it was determined that soft crib bedding was used with one infant. Evidence: One 4 month old infant was observed sleeping in a crib with a soft covering in place on top of the crib side rail. In addition, the provider had swaddled the legs and feet of this infant while sleeping.

Plan of Correction: I will remove the side rail covering. The parent specifically requested that he be swaddled when sleeping. I will ask the parent to supply a sleep sack to use instead of swaddling.
