Standard #: 22VAC40-111-120-B Description: Based on a review of caregiver records, it was determined that one file is not complete. Evidence: The file for assistant caregiver A does not include emergency contact information and documentation of her date of hire. Plan of Correction: I will have her fill out the cover sheet with this information today.
Standard #: 22VAC40-111-60-B Description: Based on a review of children's records, it was determined that three records were not complete. Evidence: 1. Child record #1 does not include: documentation to show that the parent reviewed the child's contact information within the past year. 2. Child record #3 does not include: the father's work phone number. 3. Child record #4 does not include: a signed "Medication Policy" document. Plan of Correction: I will make sure all missing information is updated.
Standard #: 22VAC40-111-500-C Description: Based on observations, it was determined that one stationary piece of play equipment was in place over a hard surface. Evidence: The backyard swing set is in place over a hard grass/dirt surface. Plan of Correction: I will install padding beneath the swings, slide, and climbing equipment.
Standard #: 22VAC40-111-530-F Description: Based on observations, it was determined that soft crib bedding was used with one infant. Evidence: One 4 month old infant was observed sleeping in a crib with a soft covering in place on top of the crib side rail. In addition, the provider had swaddled the legs and feet of this infant while sleeping. Plan of Correction: I will remove the side rail covering. The parent specifically requested that he be swaddled when sleeping. I will ask the parent to supply a sleep sack to use instead of swaddling.
