The licensing inspector conducted an unannounced monitoring inspection on Thursday, March 2, 2017 from 9:00am to 10:40am. The children were observed playing with toys upon the inspector?s arrival, and they were later observed watching a sign language video and eating snack Snack consisted of cheese sticks and raisins. The menu was posted. Today, there were four children, totaling 10 points, present with one caregiver. Two children?s records, three caregivers? records and one household record were reviewed during the inspection. Medication is not being administered and was reviewed. The center's first aid kit and emergency supplies were inspected. The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-111-730-D Description: Based on observation on 03/02/2017, the provider did not ensure that medication for household members was kept in a locked place using a safe locking method that prevents access by children. EVIDENCE: Over the counter and prescription medication was observed in an unlocked cabinet over the sink in the kitchen. The medication was out of the reach of the children in care. Plan of Correction: Removed medication today and placed in bedroom which is kept locked.
Standard #: 22VAC40-111-830-C Description: Based on a review of documentation and caregiver interview on 03/07/2017, the provider did not ensure that documentation of the emergency and shelter in place drills were maintained. EVIDENCE: The most recent documentation of an evacuation drill was dated 01/11/2017. Caregiver #1 stated that an emergency drill was conducted two weeks ago and a shelter in place drill was conducted on 03/01/2017; however these drills were not written down. Plan of Correction: Corrected on site today, documented the drills during the inspection.
Standard #: 22VAC40-111-120-B Description: Based on a review of caregiver records, and caregiver interview on 03/02/2017, the provider did not ensure that two of three records contained the required information EVIDENCE: 1. The records of Caregiver #2 & Caregiver #3, who are substitute providers, did not contain documentation of their education requirements. Caregiver #1 stated that they both have graduated from high school. 2. The record of Caregiver #3, whose date of hire is 11/23/2016, contains documentation of one reference check. Plan of Correction: 1. Will have them provide documentation today. 2. Will have caregiver provide a reference check today
Standard #: 22VAC40-191-60-C-2 Description: Based on a review of caregiver records on 03/07/2017, the provider did not ensure that two of the three records contained central registry finding within 30 days of employment. EVIDENCE: 1. The record of Caregiver #2, whose date of hire is 11/28/2016, did not contain documentation of a central registry finding. 2. The record of Caregiver #3, whose date of hire is 11/23/2016, did not contain documentation of a central registry finding. Plan of Correction: Will send off background checks today.
