Comments:
Thank you for your assistance during this unannounced renewal inspection. This on-site visit was conducted on March 6, 2017 from 11:00 a.m. through 12:30 p.m. There were no children in care. There are a total of six children enrolled in this family day home. No medication is being administered at this time. Five (5) children's records were reviewed. One provider and three (3) household records were reviewed and updated. Areas of non-compliance may be found on the violation notice. Risk scores were discussed. Your home was found to be in substantial compliance with licensing standards. Emergency preparedness plan, first aid supplies, and water/refrigerator temperature were reviewed and checked. The home is well equipped with a variety of age/stage materials for the children in care. Description: Child A did not have documentation of two emergency contacts. Evidence: Based on record review and discussion with the provider, it was confirmed Child A did not have documentation of two emergency contacts.

Plan of Correction: I will ask the parent to furnish two emergency contacts and place in Child's A record.
