Ten children were in care during the inspection, total of 24 points. The provider and one assistant were present. Children were observed during outside time and free play in the day care area. No children are on medications. Inspection time 10:00am-12:00am

Description: Based on review of six children's files not all were complete. Child A with a 2/18/16 was missing father's work address and phone number .

Plan of Correction: Corrected during the inspection.

Standard #: 22VAC40-111-200-A
Description: Based on review of two staff 's files it was not complete. Evidence: There was no documentation of staff A having orientation.

Plan of Correction: I will give the staff orientation this week.

Standard #: 22VAC40-111-210-A
Description: Based on review not all of the required 16 hours of annual training was completed. Evidence: Staff B completed four hours of annual training.

Plan of Correction: I will complete all the missed training and the annual training.

Standard #: 22VAC40-191-40-D-1-b
Description: Based on review of two staff files not all had a sworn statement prior to the first day of employment. Evidence: Staff A with a hire date of 2/18/16 did not have documentation of a sworn statement.

Plan of Correction: Staff completed a sworn statement during the inspection.
