Description: Based on observation, an infant bottle was found without a date. Evidence: One infant bottle was found with the name however it did not include a date.

Plan of Correction: Provider included the date during this inspection.

Standard #: 22VAC40-111-110-A
Description: Based on a review of the children records, one record did not include documentation of a Virginia Department of Health form. Evidence: Child #3's record, did not include documentation of a physical and immunization on the Virginia department of Health form.

Plan of Correction: Provider will give a copy of the form to the parent to have the doctor complete it.

Standard #: 22VAC40-111-690-D
Description: Based on observation of the diapering surface, the changing pad was found no longer non absorbent. Evidence: The diaper changing pad was found ripped/torn and no longer nonabsorbent.

Plan of Correction: Provider stated that she has extra and will replace the diaper changing pad.
