Discussion was held on the topics of current enrollment and current staffing.

The following inspection took place between the hours of 10 am through 12:30 pm. Today, I met with the Provider on site at her Family Day Home (FDH) located in Sterling Park, Virginia. At the start of this unannounced renewal inspection there were a total of 10 children (3 in residence) (26 points) with 3 caregivers. A total of 3 new children?s files and 8 staff/household member?s files were reviewed. The children were engaged in the following: freeplay, toy play, center play within the day care area, movie time, diaper changing, hand washing, pick up for school, lunch, and other organized activities. Standard #: 22VAC40-111-100-A
Description: Based on review, 1 of 3 new children's files reviewed indicated that the Provider did not obtain documentation of a physical examination by or under the direction of a physician prior to each child's attendance or within 30 days after the first day of attendance. Evidence - 1. On the date of inspection (03/08/2017), a physical examination for Child #6 was not available for review. 2. The start date for Child #6 was listed as 01/23/2017. Child #6 was in attendance during today's inspection.

Plan of Correction: The Parent of Child #6 shall be contacted and a current physical shall be obtained.

Standard #: 22VAC40-111-60-B
Description: Based on review, 2 of 3 new children's files reviewed indicated that not all of the children's records contained all of the required information. Evidence - 1. On the date of inspection (03/08/2017), the following child's medical insurance information was not available for review: Child #6 (policy name and number), and Child #7 (policy number). 2. The addresses for both emergency contacts of Child #7 were not available for review.

Plan of Correction: The Parents of Child #6, and Child #7 shall be contacted and the missing information placed in each applicable file.

Standard #: 22VAC40-111-590-A
Description: Based on observation and interview, an infant was not placed on their back when sleeping or napping. Evidence - 1. On the date of inspection (03/08/2017) at approximately 11:22 am., Child #8 (an infant) was observed in a crib laying on their stomach. 2. Member #2 stated (during an interview conducted on the date of inspection) that she had just laid Child #8 down, but not on their back. This was not an attempt for tummy time. 3. There was not a written statement signed by the child's physician ordering otherwise.

Plan of Correction: Corrected immediately on the date of inspection. Member was reminded on this standard and will ensure all future compliance.
