Inspector spoke with provider about 22 VAC 40-111-210 during this inspection.
An unannounced monitoring visit and home tour was conducted from 2:20pm-2:55pm. At the time of the inspector's arrival there were 7 children ages 13mos-5yrs, totalling 16 points in care with the licensee. Children were observed participating in free play, snack, nap and diapering/handwashing. A sample of 3 children's records and 1 staff record and 1 household member record were reviewed. Medication is not administered at this site. There were no reported injuries since the last inspection. Areas of noncompliance are identified on the violation notice and were discussed with the provider during the exit meeting at the conclusion of this inspection.
Standard #: 22VAC40-111-90-C Description: Based on record review it was determined the family day home did not obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: The records for child #2 (DOB 7/2/14) and child #3 (DOB 8/21/15) did not contain current immunization documentation. The documentation available was dated 1/4/16 and 8/26/16. Plan of Correction: Provider will request updated immunization documentation from the parents at pick-up.
