An unannounced renewal study was conducted with provider. There were four day care children in care, ages 6 months, 11 months, 22 months, and 1 year. The children were having rest time during today's inspection. Four children's files were reviewed - please ensure that all contract information is up to date - some still lists the old address and nearby hospital information. Two cribs reviewed are in compliance (dated 11/2015). Provider does not administer medication - Medication Administration Training is required to administer medication to day care children. First aid and CPR certification was renewed 2/11/2017. One child was picked up and the provider loaded the remaining three children in the care to pick up two school agers. Standard #: 22VAC40-111-60-B
Description: Based on review of four children's files, the provider did not have all of the required information in each child's file. Evidence: Child A and Child B did not have written authorization from the parent to obtain emergency medical care should an emergency occur and the parent cannot be located immediately. Child D did not have documentation of the name, address, and phone number for two emergency contacts.

Plan of Correction: Provider will request the information from the parents.

Standard #: 22VAC40-111-70-A
Description: Based on review of four children's records, the facility failed to give parents written information as required before the child's first day of attendance. Evidence: Child B, Child C, and Child D did not have a signed liability insurance declaration detailing whether or not the provider has liability insurance. Child D did not have a signed VDSS model form "Information and Agreement" or an equivalent. Child D did not have a VDSS model form "Provisions of the Emergency Preparedness Plan" or an equivalent.

Plan of Correction: Provider will request this information from the parents.

Standard #: 22VAC40-111-540-B
Description: Based on observation, the facility had two walkers in day care area. Evidence: Licensing inspector observed two walkers in the downstairs day care area.

Plan of Correction: Provider will remove the infant walkers from the day care and will not use them for day care children.

Standard #: 63.2-1809-A
Description: Based on review of four children's files, the provider failed to have documentation of each child's proof of identity within seven days of enrollment. Evidence: Child A, Child B, Child C, and Child D did not have documentation of having seen the proof of identity.

Plan of Correction: Provider will request this information from the parents again.
