An unannounced monitoring inspection was initiated today at 3:15pm. The physical plant, 5 caregiver/household member files, 4 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected today. There were 6 children, totaling 18 points, under the supervision of 2 caregivers. During the inspection, children were observed resting at nap time, cleaning up for and eating afternoon snack, watching an educational video and playing with puzzles. The interactions between the children and caregivers were seen to be positive and constructive in nature. Today, the children were served chicken nuggets, fries, yogurt, strawberries, blueberries and milk. The Provider is currently certified in MAT (medication administration training), CPR and First Aid. Standard #: 22VAC40-111-880 Description: Based on observations, the posted menu does not include the most recent, age-appropriate nutritional requirements of the USDA for all meals served to children in care. Evidence 1. The menu has the following items served for lunch today: chicken nuggets, fries, yogurt and milk. This does not include 2 fruits and/or vegetables, which is required by the USDA requirements. However, the Provider stated that the children were served mixed berries (strawberries and blueberries). Plan of Correction: The Provider will make sure the menu reflects all items that are served to the children for meals.
Standard #: 22VAC40-111-120-B Description: Based on file review, 1 out of 2 assistant records does not have all required information on file. Evidence 1. Caregiver #2 does not have the following information: - Name address and telephone number of a person to be notified in an emergency - Documentation that two or more references as to character and reputation as well as competency were checked before employment. Plan of Correction: The Provider will give caregiver #2 paperwork to complete (which will include emergency contacts). She will also complete reference checks.
Standard #: 22VAC40-111-200-A Description: Based on file review, there is no documentation to verify that caregiver #2 received orientation training by the end of their first week of assuming job responsibilities. Evidence 1. Caregiver #2 began caring for children on January 2017. There is no written orientation training. Plan of Correction: The Provider reviewed orientation training with caregiver #2, but did not document it. This form will be completed and kept on file.
